Forms and Waivers
Required Forms
The following forms are required for all campers before participating in camp activities. Only a parent can sign the waiver and medical history forms, so if a parent is not present on the first day of camp, it is important to be sure to complete the forms online in advance, or print them and send them along with the camper on their first day of camp. Verbal authorization or signatures from anyone other than a parent or legal guardian are not acceptable alternatives. Legal guardians, other than parents, must provide a certified copy of their power of attorney. Submit your completed forms online in advance or by fax at (858) 488-9625, or email at info@watersportscamp.com.
